AfterCare

ADULT PSYCHIATRIC INTENSIVE OUTPATIENT PROGRAM

The Intensive Outpatient Program (IOP) is an adult day program that is designed to provide clients with individualized intensive treatment for psychological disorders. Through comprehensive education and interactive group therapy the client will focus on the physical, psychological, spiritual and social features of the illness.

IOP focuses on issues of:

  • Depression
  • Medication
  • Anxiety and Panic
  • Stress Management
  • Grief & Loss
  • Anger Management
  • Relationship Conflicts
  • Co-Dependency
  • Mental Health

This intense intervention is intended to stabilize and prevent decompensation that could lead to 24-hour care or hospitalization. Program schedule is 3 hours a day, 3 days a week for ten weeks.

IOP is under the direct supervision of a psychiatrist. The multidisciplinary team consists of an MD, a registered nurse, and therapists.

The client will have an Initial Psychiatric Exam and medication management is addressed, if necessary. Medication review and effectiveness will be continually monitored. Included in the program are ongoing treatment team meetings to assess individualized goals, progress and needs of each client. Participants and family members are encouraged to engage fully in all aspects of the program. We promote meaningful family involvement and provide multifamily group sessions.

The program also offers a Dual Diagnosis track for those individuals struggling with chemical dependency and substance abuse. Highly skilled professionals provide a structured drug free environment that promotes recovery and growth.

Personalized referrals to community resources will be provided for each client prior to discharge. This will include medical management, support groups, individual therapists, and educational providers as deemed necessary.

The goal of the IOP is to optimize recovery and return the client to a healthier way of living. Through education and group therapy the client will develop new tools and abilities to cope effectively and enjoy life again.

 

CHEMICAL DEPENDENCY INTENSIVE OUTPATIENT PROGRAM

The Chemical Dependency Intensive Outpatient Program (CDIOP) is a California State Certified adult day program that is designed to provide clients with individualized intensive treatment for chemical dependency. CDIOP offers substance abuse treatment for all forms of addiction, including: opiate, methamphetamine, heroin, alcohol, and marijuana. Through comprehensive education and interactive group therapy the client will focus on the physical, psychological, spiritual and social features of the illness.

CDIOP is under the direct supervision of a psychiatrist. The multidisciplinary team consists of a physician, a registered nurse, therapists and chemical dependency specialists. Highly skilled professionals provide a structured drug free environment that promotes recovery and growth. This intense intervention is intended to stabilize and prevent decompensation that could lead to 24 hour care or hospitalization.

Program schedule is 3 hours a day, 3 days a week for ten weeks. This allows for clients to continue work, school and other responsibilities while working through their recovery and maintaining sobriety in a structured setting.

The goal of the CDIOP is to help the client develop a foundation for healthy living. The individual will build skills, behaviors and strategies that will promote a life of sobriety and autonomy.

 

NEW LEAF ACADEMY

New Leaf Academy is an adolescent chemical dependency intensive outpatient program. This program is a California State Certified day program that is designed to provide teens and youth with individualized treatment for chemical dependency or substance abuse. Through comprehensive education and interactive group therapy the youth will focus on the physical, psychological, spiritual and social features of the illness.

New Leaf Academy is under the direct supervision of a psychiatrist. The multidisciplinary team consists of a MD, a registered nurse, therapists, and chemical dependency specialists. Highly skilled professionals provide a structured drug free environment that promotes recovery and growth.

The treatment schedule allows youth to continue school, work and other responsibilities while working through their recovery and maintaining sobriety in a structured setting.

Included in the program are ongoing treatment team meetings to assess the individualized needs of each client. The client will be expected to participate fully in all aspects of the program. Accountability to the program and to one’s own recovery leads to successful outcomes.

Personalized referrals to community resources will be provided for each client prior to discharge. This will include medical management, support groups, individual therapists, and educational providers, as deemed necessary.

The goal of New Leaf Academy is to help the adolescent develop a foundation for healthy living as the client will engage in skill building, identifying behaviors and developing strategies that will promote a life of sobriety and autonomy.

 

AFTERCARE

Therapeutic Aftercare is available to participants who have completed the IOP, CDIOP or New Leaf Academy programs. Additionally, PCC offers Aftercare to individuals who have completed a higher level of care through other treatment facilities outside of the area and need additional support in their recovery journey as they return home.

Aftercare supports further discovery of self, processes triggers and focuses on relapse prevention. Participants should have motivation towards continuing recovery and an ability to engage with others in the recovery process.

Therapeutic Aftercare meets twice a month, in the evening, to support and encourage those who have a foundation in recovery and mental health awareness. Aftercare is beneficial in many areas, including:

  • Bridging the gap between completing treatment and integrating fully back into work, school or other life responsibilities.
  • Providing emotional support for day to day living through the ups and downs of post-acute withdrawal.
  • Supporting participants in maintaining and building connections with others who are in recovery or are living with mental health issues.
  • Addressing issues such as: sponsorship, conflict resolution, goal setting, motivation and healthy holiday survival.
  • Further developing recovery skills that include: healthy boundaries, awareness of cross-addiction, improved communication and healthy relationship building.

Aftercare is held the first and third Tuesday evening from 5:30  – 7:00.
